For Scouts interested in enrolling in the SCUBA class being held on Friday morning or Friday afternoon:

Underwater Adventures D.C. (UADC) is providing the SCUBA experience and requires the first two forms below. The first form, Discover SCUBA UADC, includes the PADI Medical Questionnaire and liability release agreement. The second form, the DSD Registration Form, is also required. If any of the questions on the PADI Medical Questionnaire are answered YES, then the participant MUST ALSO complete the third form, Medical Statement, which MUST be reviewed and signed by a physician. The camp nurse cannot sign this form, and the camp medical form you are already bringing to camp will not be sufficient. If you answer NO to each question on the PADI Medical Questionnaire, the Medical Statement is not required.
